Nobody walks into their first IVF consultation fully prepared. You can read everything, scroll every forum, watch every documentary and still find yourself blindsided by the waiting, the injections, the emotional whiplash, and the sheer number of things that can change between day one and a positive pregnancy test.
IVF is the most effective fertility treatment available. It is also one of the most physically demanding, financially significant, and emotionally complex experiences a woman can go through. The women who navigate it best are usually the ones who were told the truth upfront not a sanitized brochure version, but the real picture.

What IVF Actually Is
In vitro fertilization (IVF) is an assisted reproductive technology in which eggs are retrieved from the ovaries, fertilized in a laboratory, and the resulting embryos are cultured for several days before one is transferred into the uterus.
IVF bypasses several natural steps that can go wrong fertilization happens in a lab rather than inside the fallopian tube, and embryos can be assessed for quality before transfer. It is used for blocked fallopian tubes, male factor infertility, ovulation disorders, endometriosis, unexplained infertility, and age-related decline. It is also how frozen eggs are eventually used: thawed, fertilized, and transferred.
The IVF Process: What Actually Happens
A single IVF cycle takes approximately four to six weeks from the start of medications to pregnancy test.
Step 1: Suppression and baseline
Many protocols begin with birth control pills or a suppression medication to sync the cycle. At your baseline appointment typically day two or three of your period a transvaginal ultrasound and blood tests confirm your ovaries are ready.
Step 2: Ovarian stimulation (approximately 10–14 days)
Injectable FSH and LH medications are administered daily to stimulate the ovaries to produce multiple follicles simultaneously. This phase requires real schedule flexibility early morning monitoring appointments are non-negotiable and medication timing is precise. Most women experience bloating, abdominal pressure, breast tenderness, and fatigue. Ovaries stimulated during IVF can grow to the size of oranges. The discomfort is real and it is temporary.
Step 3: Trigger shot and egg retrieval
When follicles reach the target size typically 18 to 20 mm a trigger shot initiates final egg maturation. Retrieval is scheduled precisely 35 to 36 hours later. The retrieval is a minor outpatient procedure under sedation, typically 20 to 30 minutes, and most women go home the same day.
Step 4: Fertilization and embryo development
Eggs are fertilized in the lab either by mixing with sperm or via ICSI (intracytoplasmic sperm injection, where a single sperm is injected directly into each egg). Embryos are cultured for five to six days to reach blastocyst stage, which has higher implantation rates. This phase involves significant attrition: not every egg is mature, not every mature egg fertilizes, not every embryo reaches blastocyst. This is normal and one of the most emotionally difficult realities of the process.
Step 5: Preimplantation genetic testing (optional but recommended)
PGT-A (preimplantation genetic testing) biopsies embryos to identify chromosomally normal ones before transfer, reducing failed implantation and miscarriage risk. Results take one to two weeks. It adds $3,000–$6,000 but is increasingly standard of care for women over 37.
Step 6: Frozen embryo transfer (FET)
A frozen embryo transfer is now the most common approach the uterus is prepared with estrogen and progesterone and one embryo is transferred in a simple, typically painless procedure requiring no anesthesia. FET allows the body to recover from stimulation hormones and tends to produce equal or better outcomes than fresh transfers.
Step 7: The two-week wait
The ten to fourteen days between transfer and blood test are widely described as the hardest part of IVF. Progesterone supplementation continues throughout and because progesterone causes many of the same symptoms as early pregnancy, every twinge is both hopeful and inconclusive.
Step 8: Pregnancy test (beta hCG)
Ten to fourteen days after transfer, a blood test measures hCG levels. A positive result requires continued monitoring: two rising hCG values two days apart, then an ultrasound at six to seven weeks to confirm a viable pregnancy.

Cumulative success matters. A single 25% cycle does not mean a 75% chance of never succeeding. For a woman aged 38, three cycles can bring cumulative live birth odds above 60%. IVF is often as much a diagnostic process as a treatment the first cycle tells your provider how your body responds.
The attrition reality: Of 10 eggs retrieved, six or seven are typically mature; five or six may fertilize; three or four may reach blastocyst; one or two may test chromosomally normal. This is why women over 35 often need more than one retrieval cycle attrition is not failure, it is the biology of the process.
Donor eggs change the math entirely. For women over 42 or with significantly diminished ovarian reserve, donor eggs reset success rates to the donor's age typically 50%+ per transfer because egg age, not uterine age, drives IVF outcomes.
What Women Wish They Had Known
The waiting is its own kind of hard. IVF involves waiting at almost every stage: for your period, for follicles to grow, for fertilization results, for embryo grading, through the two-week wait, for biopsy results. Many women find the psychological management of waiting more demanding than the physical aspects.
Not every egg becomes an embryo and not every embryo becomes a baby. Women who retrieve 12 eggs may end with one normal embryo. Understanding attrition before you start means you are less blindsided when the numbers shrink.
One cycle may not be enough. The majority of IVF success stories involve more than one cycle. Going in with a multi-cycle mindset both financially and emotionally protects you from feeling like a single failed cycle means the door is closed.
The injections are manageable but they are real. Daily self-injections for 10 to 14 days, plus progesterone injections after transfer, are a genuine physical demand. Most women report that the anticipation is worse than the reality; learning technique from an IVF nurse beforehand helps enormously.
Your partner needs support too. Partners feel helpless, grieve differently, and are often expected to provide stability without much acknowledgement of their own experience. Couples who talk openly about how they each handle stress and build in deliberate connection outside of fertility conversations report better relationship outcomes.
A failed cycle is a real loss. The grief after a negative test following weeks of injections, monitoring, financial investment, and hope is significant and deserves to be named, not just medically filed. Giving yourself explicit permission to grieve before pivoting to the next cycle is essential for the emotional stamina IVF requires.
Your mental health is part of your treatment. Research shows that chronic untreated stress and anxiety affect fertility outcomes. Asking for a referral to a reproductive mental health counselor is not a sign of weakness it is one of the most clinically sound decisions you can make.
